Exogenous ketones: what are they, and what are they for ?

What is a ketone ?

When you run low on carbs, your liver makes ketones from fat. The main one is called BHB (beta-hydroxybutyrate). It's a fuel your brain and muscles love.

The catch : to make it yourself, you have to cut carbs for days. That's where exogenous ketones come in — "exogenous" simply means "coming from outside."

Ketosis: 3 paths

Being in ketosis simply means having enough ketones in circulation (around 0.5 mmol/L and up). You can get there three ways :

  1. The ketogenic diet — very few carbs, over several days.
  2. Fasting — your body draws on its reserves.
  3. Exogenous ketones — BHB directly, in just a few minutes.

BHB: not just a fuel

This is where it gets fascinating. BHB is also a signaling molecule: it talks to your body. Researchers are taking a close look :

  • Efficient energy — BHB bypasses a step in sugar metabolism (glycolysis) and supplies energy very efficiently.
  • Inflammation — it helps modulate certain inflammatory responses (via the NLRP-3 complex).
  • Brain — it's associated with BDNF, a factor linked to brain plasticity (its ability to rewire itself).

Sugar gives you a "high" followed by a crash. BHB, on the other hand, aims for plateau energy — steady, without the drop.
